Fat Transfer / Lipofilling

With age, or as a result of illness or accidents, some areas of the body or face may lack volume and shape. An excellent solution in this case is fat transfer (lipotransfer) or fat filling (lipofilling) using various methods — e.g. the Puregraft method. Lipotransfer alone is usually associated with approx. 30–40% loss of volume of the transplanted adipose tissue in the early postoperative period. Using adipose tissue processing systems (i.e. Puregraft and others) allows for greater predictability and durability of the result.

Before the surgery:

Before a fat transfer, a consultation is necessary during which the patient presents their expectations. If they qualify for the procedure, the source of adipose tissue harvesting is also discussed (an excess is therefore required). It is important that the patient has fat deposits from which adipose tissue can be harvested for transplantation. Before the operation itself, a series of tests for intravenous anaesthesia must be performed.

Please remember about an ECG (for patients over 40 years of age) and a breast ultrasound (in the case of fat transfer to the breasts), which is essential for performing this procedure.

Type of anesthesia:

Local or intravenous.

Duration of the surgery:

Approx. 2–3 hours.

Course of the surgery:

To perform a fat transfer, adipose tissue is harvested from a selected site and prepared for transplantation using the Puregraft system or another (depending on needs). The fat is then implanted into the chosen part of the body. Since approx. 30–40% of the transplanted adipose tissue disappears, we perform overcorrection — an intentional, excessive increase in volume. This volume decreases after a period of 4–6 weeks.

Postoperative recommendations:

After fat transfer, wearing a compression garment at the harvesting site continuously for one week and as often as possible for one month is recommended. The first bath at the operated site is possible 2 days after the drainage from the harvesting wounds has stopped — usually on day 4 or 5 after the procedure.
Additionally, massages of the harvesting area are recommended (can begin from day 7–10 at the earliest). At least 10 massages (endermologie) over 5–6 weeks should be performed (maximum 1–2 massages per week). Carboxytherapy is also recommended in parallel, as it will improve blood supply and skin metabolism.

Bruising after fat transfer at the injection site may persist for approx. 2–3 weeks. The operated site (i.e. where the fat was transplanted) must absolutely not be massaged, squeezed or compressed. Arcalen ointment to speed up bruise absorption and/or Cebelia cosmeceuticals to support healing — reducing swelling and bruising — may be used.

Recovery time:

The final result after a fat transfer procedure is visible after 3 months.
